Children's Medical Report

Parent completes the medical history section. Your child's doctor completes the physical exam (Part B) before enrollment.
Shaken Baby / Abusive Head Trauma Policy Acknowledgment

Acknowledgment that you received the facility's SBS/AHT prevention policy (one per family).
Discipline & Behavior Management Policy
